Home and Community-Based Services

Night Supervision

Our night supervision service provides you with a sense of security and safety during the night hours. We ensure that awake staff is always present to monitor your needs, assist with any nighttime tasks, and provide support in case of an emergency.

 

IHS with/without Training

Our Individualized Home Support (IHS) can be provided in the person's own home or in public community settings. Our trained and experienced staff can assist you with IHS tasks such as medication reminders, tracking your health, communicating with your healthcare provider to set medical appointments, behavioral management, activities of daily living ADL'S, promote community integration. We also offer training services to ensure that you and your loved ones can take control of your health and wellness.

Respite

Taking care of a loved one can be a challenging task, and you deserve a break. Our respite services allow you to take time off while we take care of your loved one. Our experienced staff can provide assistance with daily activities, provide companionship, and ensure that your loved one is safe and comfortable.

Homemaking

Our homemaking services allow you to maintain a clean and organized home. Our experienced staff can assist with tasks such as laundry, meal preparation, light housekeeping, and shopping, ensuring that you have a comfortable and safe living environment.

ICLS

Individual community living supports (ICLS) is a bundled service that includes six service categories. ICLS services offer assistance and support for people who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own homes. Individual community living supports (ICLS) can also be delivered through remote support.

Employment Services

The state of Minnesota added three employment services to our home and community-based services (HCBS) waivers on July 1, 2018. We offer employment exploration, development and support services. Alfa Care Services offers individualized services designed to help a person achieve competitive, integrated employment, become employed in their community.
